• Use of CKd App
  • “Care for Kidney (CKd)” is a free health application designed to promote kidney health and provide education on delaying the progression of chronic kidney disease (CKD).
  • This application does not provide professional medical diagnosis, advice or treatment. The pre-set targets in the “Self Monitoring” page are based on existing guidelines for the general population aged 18 years old and above. DO NOT use the system if you are under 18 years old. Where possible, targets should be individualized. Please consult your doctor regarding your appropriate targets.
• Risk and Liability
  • All parties involved in this project namely the lead author, contributors, sponsor and developers are referred as “the team”.
  • Whilst the team is committed to ensure the safekeeping of your data to maintain data confidentiality, security and integrity, the use of this application is strictly at your own risk.
  • The team will not be held responsible and liable for any errors or omissions in this application; and in respect of any consequences, losses and damages arising therefrom.
  • The team makes no representation to the availability, accuracy, liability, completeness, quality, performance or timeliness of the application, services and content. The team does not warrant:
    • That the application will be available and running continuously without interruption
    • That the application or hosting content are free from any external security threats such as virus, Trojan horse, worms or other malicious codes or properties
    • That the application will run in every version of mobile phone or be compatible with all the hardware or software in all types of devices
    • That all defects will be corrected
    • To provide technical support or maintenance for the application

 

• Privacy Policy
  • Any personal data and health information collected from you will be used to auto-calculate estimated glomerular filtration rate (eGFR) and to provide personalized feedback.
  • The data is owned by National Kidney Foundation (NKF) Malaysia, and may be used for research purposes. All information collected will be anonymized without revealing your personal information.
  • Your data will be shared with the healthcare providers in the centre(s) that you grant access to from the CKd Application under the ‘My Doctor List’ section.
  • No personal information will be shared with any third parties other than for the limited purposes as described below:
    • As required by law of Malaysia, or similar legal process
    • When disclosure is necessary to protect our rights, protect your safety or the safety of others, investigate fraud or respond to a government request.
    • Sharing with our contractors or consultants that provide us developmental and maintenance services, provided they are bound by rules of confidentiality.

 

•  Termination
  • You may at any time uninstall the application if you wish to cease using.
  • If you violate this agreement, we may at our sole discretion terminate your access to the application.
