“Care for Kidney (CKd)” is a free health application designed to promote kidney health and provide education on chronic kidney disease (CKD). It is a trilingual app – English, Malay and Chinese. We aim to encourage self-management and lifestyle changes with the use of this application.
Key features of the “CKd app”:
- Learn about chronic kidney disease (CKD)
- Allow you to monitor your blood pressure, glucose and weight on a regular basis
- Compare your measurements with targets set by your doctor
- Allow you to record your blood and urine results such as kidney profile, HbA1c, cholesterols, urine albumin/ protein and CKD-related parameters
- Export your results into a PDF report
- Remind you to take medications at the right time with alarms set at regular intervals
- Allow you to add photo to each medication for easy identification
- Provide latest update and tips on kidney health for educational purpose
- Allow for shared management with other medical specialties managing you
This app does not provide professional medical diagnosis, advice or treatment. The pre-set targets under the “Patient’s targets” section are based on existing guidelines for the general population. Where possible, targets should be individualized according to your factors and risks. Thus, please consult your doctor regarding your appropriate targets.
